Treatment Questions

Physical therapists are important partners in your health. As experts in the way the body moves and as highly educated health care professionals, physical therapists help you achieve higher levels of function and improve quality of life without surgery or opioid prescription medication, in many cases.

We offer free 15 minute consultations! Call us to schedule a consultation or stop in any time during business hours.

Physical therapists graduating today are required to have a Doctorate degree and pass national and state licensure examinations before being allowed to practice. You can expect the best professional care from our highly trained and qualified staff.

Physical therapists understand that the road to recovery is unique and personal, so you will always receive a specialized treatment plan from us tailored to your individual needs. We recognize that you are the most important member of your own health care team. We work with you one-on-one to meet your specific needs.

Visits typically last 60 minutes depending on the treatment being provided and our appointments are scheduled on the hour.

We make every effort to ensure that you are seen by your primary therapist, but we take a collaborative approach to make sure you have the right therapist for your care. You may be seen by another therapist from time to time if it benefits your plan of care.

Your first visit will include an evaluation and, time permitting, your treatment will be started. Please bring your insurance information with you. We will need to make copies for our records.
 
Recommended Clothing: Please wear loose, comfortable clothing.
For Spinal Injuries, Knee, Ankle, Hip Issues: Gym shorts, comfortable/loose tops.
Arm, Shoulder Issues: Gym shorts, tank top/loose top.

Billing & Insurance Questions
Not all insurance companies require a referral in order for services to be covered. We will assist you in determining if your insurance does require a referral for treatment. For Medicare recipients, a medical professional must sign off on your plan of care within 30 days of your treatment.
 

If you do not plan to use insurance, you may see a Physical Therapist without a referral on a cash basis. Please note for all private pay customers, payment is due at the time of service, unless otherwise agreed upon.

Absolutely! You have the right to choose your healthcare provider. We can assist in notifying both your referring provider and insurance company of your choice to switch to Optimum Health & Wellness.

Most insurance plans will cover your services at  Optimum. Please call the front desk to request a verification of your benefits if you would like the details of your plan’s coverage prior to starting physical therapy or occupational therapy.

  • A quote of benefits and/or authorization from your insurance carrier does not guarantee payment or verify eligibility. Payment of benefits are subject to all terms, conditions, limitations, and exclusions of the member’s contract at time of service.

Yes, with authorization of your physical therapy plan of care by your healthcare provider. All current Medicare Rules must be complied with during your care with us. We will also bill for you any secondary (supplemental) insurances.

Each insurance plan’s benefits can vary. Depending on your coverage, you may be responsible for a co-pay or the balance on your account. You may contact our Medical Biller to find out exactly what your plan will cover.
